It's good to have a spare tire although they are become increasingly rare. I've used my Sienna's spare tire several times including while waiting several days for a replacement for a road damaged tire to arrive. That said, it's likely that our two currently owned Toyotas will be the last vehicles we own that have spare tires.
A 145/85R18 spare size is within the accepted tolerance range for temporary use with 2019 Toyota Sienna OEM tire diameters. The 18" spare wheel will not interfere with the brake or suspension system, regardless of OEM wheel option. The vehicle can have a different wheel size than the spare and it will still work properly.
